Provider must call (303) 789-9360 and receive a
“Return Authorization” number
from an official Torii Medical™ Customer Service
Representative. Although not required, we recommend
that you ship the product with “delivery
confirmation” from USPS, UPS, FEDEX
or other carrier as we are not responsible
for products lost in shipping.

Provider must include a letter or note stating the reason
for the return, along with the return
authorization number included with the
return package.

is responsible for returning product to
the Torii Medical™ facility including paying for
all return shipping and/or handling charges.
Product will be inspected prior to authorizing
exchange, return or refund.
